Setting new marathon records: how routine physiotherapy supports endurance performance
The runners who improve are usually the ones who keep showing up. Here is what the evidence says about staying healthy enough to train — and where a physiotherapist fits in.

Elite marathon times keep falling, and it is tempting to credit the shoes, the supershoe foam, or some secret session. But for almost every runner — from a first-timer chasing a finish to a club athlete chasing a personal best — the single biggest lever on performance is far less glamorous: staying healthy enough to keep training. Fitness is built by the sessions you actually complete, week after week, and the fastest way to stall progress is to spend six weeks nursing an angry Achilles instead of running.
That is where routine physiotherapy earns its place in a training plan — not as a rescue service for the already-injured, but as part of how consistent runners stay consistent. This article summarises what the research actually shows about running injuries, strength work, gait, and load. Running is remarkably good for you — and remarkably injury-prone
Running is one of the most accessible forms of exercise, but the repetitive loading that makes it effective also makes it hard on tissues. Every kilometre is thousands of ground contacts, each sending force up through the foot, shin, knee and hip. Do enough of them, increase them too quickly, and something tends to complain.
Those numbers explain a lot about why so many training blocks unravel. Novice runners are especially exposed, likely because their bodies have not yet adapted to repetitive load — a reminder that building up gradually matters most exactly when motivation is highest. The encouraging flip side is that many running injuries are overuse injuries related to how training is structured, which means they are, in principle, among the more modifiable problems in sport [2].
Strength training: the best-evidenced upgrade to running economy
If there is one intervention with strong research support for both durability and performance, it is resistance training. The mechanism is running economy — the amount of oxygen (and energy) you burn to hold a given pace. Two runners with identical maximal aerobic capacity will not race the same if one is more economical; the efficient one covers the distance for less. Running economy is considered one of the key physiological determinants of distance performance, and strength training appears to improve it.
Importantly, this kind of strength work does not turn distance runners into bodybuilders or blunt their aerobic engine. The reviewed programmes typically used heavy resistance, explosive, or plyometric exercise a small number of times per week alongside running, and the endurance training remained the main event. The benefits are thought to come from stiffer, more efficient tendons and better neuromuscular coordination rather than bulk. Beyond economy, stronger muscles and tendons are also more resilient to the repeated loading that drives overuse injury — so strength work plausibly supports both halves of the performance equation: going faster and staying available to train.
What the evidence does not support is a single universal routine. The right exercises, loads and progression depend on your injury history, training phase and current capacity — which is precisely the kind of judgement a physiotherapist or strength coach makes in an individual assessment, not something to copy wholesale from an article or a social feed.
Gait retraining: subtle tweaks, measurable changes in load
How you run — your cadence, footstrike and mechanics — shapes where and how much force your tissues absorb. Gait retraining is the physiotherapy-led process of nudging those patterns, most often by modestly increasing step rate (cadence). Take slightly quicker, shorter steps at the same pace, and you tend to reduce overstriding and lower the peak impact forces travelling up the leg.
The honest summary: gait retraining is a well-supported tool for changing the mechanical variables associated with certain overuse injuries (such as some cases of shin and kneecap pain), and it does not appear to cost you speed. Whether it prevents injuries across the board is not yet settled. That nuance matters, because gait is individual — a change that helps one runner can unload one structure while overloading another. This is a clear case for a professional assessment, ideally with the runner observed in motion, rather than a blanket instruction to "just run at 180 steps per minute."
Load management: the quiet discipline behind personal bests
Ask experienced coaches what breaks runners and most will point not to a single wrong exercise but to a training error: doing too much, too soon, too fast. The body adapts to load it has seen before; it protests when the demand suddenly outruns what it is prepared for. Managing that relationship — the acute spike versus the chronic base you have built — is at the heart of injury prevention.
The practical takeaway is a principle, not a prescription: build volume and intensity progressively, respect recovery, and treat sudden mileage leaps with caution — especially early in a training cycle or when returning from a break. A physiotherapist can help interpret your own history, identify the patterns that have flared things up before, and support a return-to-running progression after a setback so the comeback doesn't simply reset the cycle.
Consistency is the real performance driver
Notice the thread running through all of this. Strength work improves economy and resilience. Gait retraining reduces impact loading. Load management prevents the spikes that derail training blocks. Each is worthwhile on its own — but their shared payoff is the same: more weeks of uninterrupted, progressive training. Endurance adaptations are slow and cumulative, and they are only banked by sessions you actually run. The athlete who trains steadily for months will almost always outperform the equally talented one whose year is a stop-start series of niggles and layoffs.
The best ability is availability — the fitness that counts is the fitness you were healthy enough to build.
That reframes what routine physiotherapy is for. It is less about fixing you after a breakdown and more about the unglamorous work that keeps breakdowns rare: screening for the loads and imbalances that tend to bite, coaching strength and mechanics, and guiding sensible progression. None of it guarantees a record — no honest source can promise a result, and this article does not. But it stacks the odds toward the thing that reliably produces improvement: showing up, healthy, again and again.
